Substitute Andy Carroll scored with virtually his first touch of the game to put Chelsea on the verge of their first league defeat under Guus Hiddink.
The former Newcastle and Liverpool striker, who got the winner when the two teams met at Upton Park earlier in the season, latched on to a Dimitri Payet pass and slipped the ball past Blues keeper Thibaut Courtois at his near post.
Seconds earlier, West Ham keeper Adrian palmed away a Loic Remy shot as Chelsea – who had been the dominant side in the second half – hunted a breakthrough of their own.
Oscar should have done better five minutes after half-time in meeting an inviting Branislav Ivanovic cross, but stretched to meet it with his right foot rather than using his left and couldn’t divert it on target.
Adrian had to be alert to punch away John Terry’s header, from a Cesc Fabregas free-kick, while Ivanovic continued to find space down the right.
West Ham did warn Chelsea of their continued threat on the break when Aaron Cresswell’s cross-cum-shot hit the bar from a tight angle.
It was a lesson the Blues failed to learn, as Payet’s pass caught Ivanovic out and Carroll pounced.
Manuel Lanzini had given West Ham a 17th minute lead when he picked up a loose ball 20 yards out and curled a brilliant right-footed shot beyond Courtois.
Fabregas levelled deep into first-half injury time with a superb free-kick from 20 yards.
